What is continuing healthcare funding?

NHS continuing healthcare means a package of ongoing care that is arranged and funded solely by the NHS where the individual has been found to have a ‘primary health need’ as set out in The National Framework for NHS Continuing Healthcare and NHS funded Nursing Care.

How to apply for funding

If you would like to apply for continuing healthcare funding or have an enquiry, NHS Dorset Clinical Commissioning Group (CCG) has a Continuing Healthcare Team based at both Vespasian House, Dorchester and Canford House, Poole.  You may contact either team at the addresses or phone numbers listed below.
