The NHS Long Term Plan says that personalised care will become business as usual across the health and care system, through implementing a Universal Personalised Care model.

“A one-size-fits-all health and care system simply cannot meet the increasing complexity of people’s needs and expectations. Personalised care is based on ‘what matters’ to people and their individual strengths and needs.” (NHS England)

The NHS model of personalised care is reflected in what we do in Dorset including:

  • a proactive and universal offer of support to people with long-term physical and mental health conditions to build knowledge, skills and confidence and to live well with their health condition
  • intensive and integrated approaches to empowering people with more complex needs to have greater choice and control over the care they receive.

We are also working with a number of Primary Care Networks to focus on taking a personalised approach to improving services.  Each area is exploring what it takes to establish and embed a personalised approach to how they work.

A key part of this will be to work in partnership with people, communities and the voluntary sector, reviewing and creating an approach together.
