We comply with Data Protection and Access to Medical Records legislation and are committed to assisting you in accessing personal information that we hold on you.

The Data Protection Act 1998 gives everyone the right to see, or have a copy, of any personal information held about them.

For formal access to your records you must make a request in writing to the address below or by email following this link:

All requests may be subject to an administration fee and no information will be released without the consent of the patient, or that of an authorised representative, unless we are legally obliged to do so. 

In special circumstances the law allows us not to show you your information, such as if we consider it harmful to you, or another person’s physical and/or mental wellbeing.

You can request informal access to records by speaking to the professional staff involved in your care. 

If you wish to view or have copies of hospital records you will need to contact the Medical Records Department at the appropriate hospital.

If you wish to view or have copies of your GP medical records please contact the Practice Manager at your GP Practice.
